Tirzepatide in Thailand: the honest guide for mainstream adults

What tirzepatide is, what it costs in Bangkok, how to tell a real clinic from a mediocre one, and the questions you should ask before your first injection.

What tirzepatide actually is

Tirzepatide is a weekly injection. It activates two receptors at once: GIP and GLP-1. Both are gut hormones involved in appetite regulation and insulin response. Older drugs like semaglutide hit only GLP-1. Tirzepatide hits both, and in head-to-head trials it produces more weight loss.

The primary trial is SURMOUNT-1, published in the New England Journal of Medicine in 2022. At the full dose (15 mg weekly), participants lost around 20% of body weight over 72 weeks. Placebo participants lost around 3%. That is a real drug effect, not a trial artifact.

The drug was originally developed for type 2 diabetes. Blood sugar outcomes were the first use case. Weight loss became the second use case when researchers noticed how consistently participants lost weight as a side effect.

What it costs in Bangkok

Prices vary by vendor, channel, and what you are comparing. Rough ranges as of early 2026:

  • Branded Mounjaro through a clinic, weekly dose: 8,000 to 14,000 THB per month depending on dose
  • Branded Mounjaro through a hospital group (BDMS, Bumrungrad): usually bundled inside a wellness program, 30,000 to 60,000 THB total
  • Research-chem tirzepatide, self-dosed: 3,000 to 6,000 THB per month for vials plus peripherals

The spread is not trivial. A year of branded Mounjaro through a private clinic runs roughly 100,000 to 170,000 THB. A year of research-chem self-dosing runs 40,000 to 75,000 THB. The gap pays for medical oversight, brand-verified product, and the clinic being on the hook if something goes wrong.

Our opinion: if you have never injected anything before, pay the premium. If you have done self-administered hormones or other injectables under supervision and you know what sterile technique looks like, the research-chem route is defensible. Either is legitimate. Neither is universally correct.

What to ask a clinic before your first visit

Five questions. If they cannot answer any of them clearly, move on.

1. Is the product branded Mounjaro or compounded? Both exist in Thailand. Compounded is not automatically worse, but it is a different conversation. 2. What is your cold chain policy from receipt to dispensing? Tirzepatide is stable at 2 to 8 degrees Celsius refrigerated. It tolerates brief excursions. A clinic that says "temperature is not a concern" has disqualified itself. 3. What dose titration do you use? The standard is 2.5 mg weekly for four weeks, stepping up to 5 mg, then 7.5, then 10, then optionally 12.5 and 15. Anyone starting you at 10 is either ignoring the label or optimizing for a metric that is not your comfort. 4. What happens if I have a bad reaction at week three? The right answer involves a named clinician, a phone number, and a specific follow-up appointment. "Just stop taking it" is not the right answer. 5. What is your refund or switching policy? About 10% of people do not tolerate tirzepatide well enough to continue. A clinic that cannot articulate what happens when you are in that 10% has not thought about you as a customer.

Red flags on research-chem vendors

The research-chem market exists in a legal grey zone in Thailand. The phrase "for research use only" is a legal shield, not a barrier. Individuals buy these products and self-administer. That has been true for years.

What separates a defensible research-chem vendor from one we silent-filter:

  • Batch Certificate of Analysis (COA) from a third-party lab (Janoshik Analytical, Finnrick, and Cheminoz are the common references). No COA, no sale.
  • Cold-chain shipping for heat-sensitive compounds, with tracking. GLP-1s are heat-sensitive.
  • A real return address and a responsive human when something goes wrong.
  • Consistency. Vendor quality drifts. The ones worth recommending stay above the bar for a year at a time, not a month.

"Research-chem tirzepatide" is the same molecule, manufactured by other facilities (mostly Chinese and Indian pharmaceutical contract manufacturers), sold under "research use only" framing. Quality varies. The best research-chem vendors publish batch COAs and sometimes get product from the same contract manufacturers that supply licensed brands in other countries.
