Singapore vs Bangkok for GLP-1 medical tourism: which makes sense for whom

The two most common medical-tourism endpoints in Asia for GLP-1 access are Singapore and Bangkok. They are not the same product, even though both deliver the same molecule. Choosing badly between them costs either money or reliability depending on which axis you got wrong.

Here's the comparison that the marketing pages of either city's clinics won't run for you.

The 30-second version

Singapore: more expensive, more predictable, more English-fluent across the board, less price negotiation, fewer compounded options, faster intake.

Bangkok: cheaper, wider quality spread, more compounding options, more price negotiation, more variable supply, more options at every price point.

If you're optimizing for a single visit and time is more valuable than the price differential: Singapore.

If you're optimizing for ongoing supply and the price differential matters: Bangkok.

If you have flexibility on both axes: read the rest.

Pricing reality

Brand Mounjaro at a typical mid-tier private clinic:

• Singapore: roughly 1,400-1,800 SGD (~33,000-42,000 THB equivalent) per month at 5mg, plus 200-400 SGD consultation
• Bangkok: roughly 9,500-12,000 THB per month at 5mg, plus 800-1,500 THB consultation

Singapore is 3-4x the price for the same drug at the same dose. The price difference reflects clinic overhead, regulatory environment, and the willingness to pay of the local market.

Compounded tirzepatide:

• Singapore: rarely available. The compounding ecosystem in Singapore is more constrained than in Thailand, partly by HSA (Health Sciences Authority) regulatory practice, partly by the smaller market.
• Bangkok: 3,500-7,500 THB per month at 5mg-equivalent for legitimate compounded options. Wide quality spread.

If compounded is the price tier you're aiming for, Singapore mostly isn't an option. Bangkok is.

Reliability and supply

Singapore: Brand Mounjaro stock is generally reliable. The supply chain is shorter (drug imports from Lilly's regional distribution), inventory turns are predictable, and out-of-stock events are rare and short.

Bangkok: Stock varies by clinic and by month. A premium hospital is reliable. A mid-tier clinic can run out for 1-3 weeks at a time. A research-chem-leaning operator can lose access to a supplier with no warning.

If your trip is short and your timeline is fixed (you fly Tuesday, leave Friday, want a 4-week supply in hand), Singapore is more dependable. If your timeline has flex and you can adjust your visit by a week or buffer through inventory swings, Bangkok works.

Quality assurance

Singapore: HSA enforcement is stricter than Thailand FDA equivalent. Authentic brand pharmaceuticals are essentially universal at registered clinics. Counterfeit risk is very low.

Bangkok: Premium hospitals are equivalent to Singapore on authenticity. Mid-tier clinics are mostly authentic but require some vendor due diligence. The further down the price tier you go, the more verification work falls on you. The Panya rubric does most of this filtering at scale.

A Singapore-side clinic visit is closer to a "trust by default, verify if curious" posture. A Bangkok-side clinic visit at lower tiers is closer to "verify by default."

English fluency and clinical workflow

Singapore: English is the working language of medicine. Every clinic intake form, every prescriber conversation, every pharmacy interaction is in English. Communication friction is approximately zero.

Bangkok: Premium hospitals are equivalent to Singapore on English fluency. Many mid-tier clinics have at least one English-fluent prescriber but the pharmacy and intake staff vary. Some clinics that look English-friendly online are functionally Thai-only when you arrive.

For a tourist with no Thai, Bangkok requires picking the clinic intentionally for English support. Singapore doesn't.

Time to first prescription

Singapore: Walk-in or same-day appointment is feasible at most private clinics. From appointment booking to prescription in hand: usually 1-3 hours total. From arrival in country to prescription: same day.

Bangkok: Same-day appointment is feasible at premium hospitals. Mid-tier clinics often want a 24-48 hour booking window. From appointment booking to prescription in hand: 1-4 hours including travel between clinic and pharmacy. From arrival in country to prescription: same day at premium tier, next day at mid-tier.

For a 24-48 hour layover scenario, Singapore is operationally easier.

Visa and entry friction

Singapore: 90-day visa-free for most Western passports. Easy entry, no health screening, no medical-tourism-specific documentation needed.

Bangkok: 30-90 day visa-free or visa-on-arrival for most Western passports. Same low friction. No medical-tourism-specific paperwork required for personal-use prescriptions.

These are roughly equivalent.

When Singapore is the right call

• You have a one-time visit and time is the binding constraint
• You want the strongest authenticity guarantee available in Asia
• English-fluency matters for any reason (medical complexity, language preference)
• You're not coming back for ongoing supply; this is the only visit
• Brand Mounjaro is your specific goal and price is secondary

When Bangkok is the right call

• You're prepared to invest in choosing a clinic and verifying it
• The price differential of 3-4x is meaningful for your situation
• You're optimizing for ongoing supply rather than a single transaction
• You're comfortable with mid-tier clinic workflow
• Compounded options are on the table for you
• You want the wider Bangkok-specific GLP-1 ecosystem (longevity clinics, regular followup, etc.)

When neither is right

If you're looking for budget compounded tirzepatide at the absolute lowest price, Vietnam is cheaper than Bangkok. If you're looking for a Western-regulated environment, your home country private telehealth is probably still better than either despite the higher cost. The medical tourism arithmetic only works at certain price-time-reliability combinations.

What I'd actually recommend

For a first medical tourism trip with no GLP-1 history: Singapore. The predictability is worth the premium. You'll learn the protocol in a low-friction environment. If you decide it's worth doing again, you can move to Bangkok for the second trip.

For a returning user who's already had one GLP-1 prescription anywhere and just needs supply: Bangkok. The 3-4x cost difference compounds across multiple trips. The Bangkok mid-tier clinic ecosystem is reliable enough once you've picked a clinic.

For an expat in either Singapore or Thailand: stay in your home city. The economics of cross-border medical tourism rarely beat your local market once you've picked a good clinic locally.

The cross-regional comparison that often works against people: optimizing for the lowest single-visit price without accounting for whether you can actually trust the clinic on the second visit. Trust is a multi-visit construct. Bangkok rewards trust-building; Singapore arrives with trust pre-loaded.
