Mounjaro vs Wegovy: which one for me?
For most weight-loss patients, Mounjaro (tirzepatide) is the better first choice if you can get it: higher average loss, similar tolerability. Wegovy (semaglutide) wins on three specific use cases: proven cardiovascular outcomes, established T2D coverage in some markets, and supply when Mounjaro is short.

The head-to-head data
SURMOUNT-1 (tirzepatide, 2,539 participants, 72 weeks) reported 20.9 percent mean weight loss at 15 mg. STEP-1 (semaglutide, 1,961 participants, 68 weeks) reported 14.9 percent mean weight loss at 2.4 mg. The SURMOUNT-2 head-to-head extension (2024 ADA data) showed tirzepatide outperforming semaglutide by roughly 5 to 7 percentage points in matched cohorts. For pure weight loss, tirzepatide wins. Both are massively better than placebo (2 to 3 percent loss).
Side effect differences that matter
Tolerability is similar but not identical. Both produce nausea, fullness, and constipation as the main GI burden. Tirzepatide users report slightly less severe nausea on average but more pronounced food-noise reduction. Semaglutide has a longer half-life (about a week) so dose-day matters less. Severe gastroparesis incidence is similar between the two (rare for both, but real). The real differentiator is what your prescriber and your pharmacy stock; switching mid-titration is doable but costs you a re-titration cycle.
When semaglutide is the better choice
Three scenarios. SELECT (NEJM 2023, 17,604 participants) showed semaglutide reduced major adverse cardiovascular events by 20 percent in patients with established CVD and overweight/obesity. Tirzepatide does not have an equivalent published outcome trial yet (SURPASS-CVOT runs to 2027). T2D coverage: in many markets (UK NHS, parts of EU), semaglutide is reimbursed for T2D in cases where tirzepatide is not yet listed. And supply: when local Mounjaro is short, Wegovy may be the practical option even if it is the second-best on weight outcomes.
