How cold do GLP-1 pens really need to stay?
Refrigerated 2 to 8 C is the manufacturer storage spec. Once in use, most pens tolerate room temperature up to 30 C for 21 to 30 days depending on the brand. Heat above 30 C, freeze, or 30 days at room temp and the pen should be discarded. Real-world cold-chain breaks during travel are forgivable; sustained heat is not.

What the labels actually say
Mounjaro KwikPen: 2 to 8 C refrigerated, may be stored up to 30 C for 21 days once in use. Zepbound autoinjector: same. Wegovy: 2 to 8 C refrigerated, 6 weeks at up to 30 C once in use is acceptable per Novo. Ozempic: same as Wegovy storage spec. Rybelsus oral: 20 to 25 C, no refrigeration needed because it is a tablet. None of these are designed to survive freeze, and freeze causes irreversible aggregation that destroys efficacy.
What this means for travel
A few hours unrefrigerated during airport security or a flight is not a problem. A 12-hour transit in a cool bag with a frozen gel pack (do not let the pen touch the ice) is fine. Tropical heat in a parked car, or any scenario where the pen sits above 30 C for hours, is the failure mode. The cold-chain industry uses phase-change materials with a 25 C transition for exactly this reason. If your pen has been somewhere visibly hot, treat it as discarded.
How to spot a heat-damaged pen
Look at the cartridge through the window. Healthy GLP-1 solution is clear and colorless. Cloudiness, particles, or a yellow tint mean the protein has aggregated. Do not inject. Same for any pen that was clearly frozen and thawed; freeze damage is invisible in solution but the protein is destroyed. When in doubt, discard. The cost of a wasted pen is much less than the cost of three weeks of zero effect because you injected denatured drug.
