SURMOUNT-4 (NEJM 2024) randomised participants who had completed 36 weeks of open-label tirzepatide to either continue or switch to placebo for 52 weeks. Continuing arm: an additional 5.5 percent weight loss. Placebo arm: 14 percent regain on average. The takeaway is not 'GLP-1s do not work' but 'GLP-1s work for as long as you are on them.' The mechanism that drives appetite suppression and gastric slowing is pharmacological, not behavioural retraining. Stop the drug, the mechanism stops.
